You will need a launcher that supports custom icon packs, supported launchers are listed below...

nova settings --> look and feel --> icon theme --> choose Cards Icon Pack.

themes --> download button(top right corner)--> icon pack--> choose Cards
Icon pack.

long press home screen--> awd settings--> icon appearance --> underneath

HOW TO SET HOMESCREEN LIKE IMAGE
Its very easy.
Install Card Icon Pack and apply the icons.
Nova settings--> dock --> disable dock.
Nova settings--> desktop--> desktop grid--> set 5 rows and 4 columns.
Nova settings--> desktop--> icon layout--> set icon size to 120.
Download vizorg widget app and set it as your wish.
Drag useful apps to home screen and set them as your wish.
For applying wallpapers, download the app "My Wallpapers" from my play store page (its free) and i upload it on play store specially for my icon pack users.
NOTE
Please use light colour wallpapers for this icon pack, so that shadows and cut will appear
on background.
